SE Health Driver Assessment & Training is a Ministry of Transportation of Ontario approved driver rehabilitation facility which offers a wide range of services to support individuals with mobility or age-related changes.

Outpatient rehab physical therapy is designed for patients who are in need of physical therapy due to an injury or serious medical condition but are independent enough to receive these services while living at home. The focus of outpatient physical therapy is helping people improve their mobility and quality of life, so they can get back to daily activities. Visits tend to be between 40 and 60 minutes, with additional exercises that a patient can do at home. An outpatient physical therapy clinic may also offer other services such as aquatic therapy, biofeedback, massage, and more. Impact-oriented applied health services research and evaluation in four priority areas. Hands-on support with chores, meal planning and preparation, groceries, laundry, light housekeeping, transportation, social interactions and other daily tasks. Support with bathing, toileting, dressing, management of medication, feeding and other daily activities. SE Health Registered Dietitians provide diet assessments, develop nutritional plans and menus, and counsel patients to make changes to their diet. Dietitians are especially helpful for patients with cancer, diabetes, digestive, or swallowing disorders. Our dietitians can develop food modification plans to help you reduce the risk of choking, dehydration or malnutrition.
Hospice care focuses on the pain, symptoms, and stress of a terminal illness. Hospice care is provided by an interdisciplinary team overseen by Board-certified Hospice and Palliative Care physicians. This team works directly with the patient, their family members and caregivers, to provide care for the whole person in mind, body and spirit.
Fares cost $3 each way, and waivers are available if you meet the low-income guidelines. Rides are available for shopping, medical appointments and recreation, and the service can help you make various bus and rail connections. The program covers Elizabeth and all of Union County plus destinations five miles outside its borders. Out-of-county rides are available on Mondays and Wednesdays, and trips can be scheduled up to three weeks in advance.
